The Hospital in the Home (HiTH) service is a virtual ward which has been developed to provide a transition to discharge for patients with an acute/sub-acute condition deemed by a senior medical officer as appropriate for care outside the hospital ward setting.
The Service operates seven days a week between 7am and 10pm, fully supported by a Consultant and RMO.
We are looking for casual Registered Nurses to join our team by providing patients with a comprehensive and accurate nursing assessment to identify needs, risks and early warning signs that will affect care plans and interventions required.
You will do your clinical health assessments via telehealth. This includes health history, observations, risk assessment, diagnostic tests, psychosocial needs, cultural and learning needs and act proactively.
