About the Role

We have a great opportunity for a senior registered nurse with strong leadership and advanced nursing skills to join our team as a Diabetes Clinical Nurse Specialist (CNS).
The Diabetes CNS is an advanced nursing practice role that encompasses responsibility and coordination of nursing care to this patient group across health care settings – inpatient, outpatient and community.

The CNS works as part of a Diabetes Integrated Team and contributes to the development of integrated service delivery across the continuum of care in conjunction with medical, allied, and non-clinical support roles. The CNS works closely with the Diabetes Integrated Team to develop evidence-based policies and procedures; and improves the experience for patients and their family/whanau.

The CNS works closely with General Practitioner (GP) Services and partner organisations (Tui Ora, Ngāruahine Iwi Authority, Ngāti Ruanui Tahua Ltd and Pinnacle Midlands Health Network).

About the Team and Location

The Taranaki Diabetes Integrated Team has two primary functions:

1. Comprehensive Clinical & Psychosocial Intervention - Providing clinical expertise and psychosocial support to directly manage patients with diabetes with the aim of discharging them back to primary care as appropriate.

2. Enhancement & Capability Support - Provide support, education and training for both health professionals and patients, guided by the needs of primary care and Māori providers, to ensure patients with diabetes receive high quality and consistent clinical care and health information.

About you

The successful applicant will have:

· New Zealand Nursing registration with current Annual Practicing Certificate

· Four years post graduate experience and a sound knowledge base in Diabetes Management.

· Completed post graduate study in Long Term Condition Management, Diabetes Disease Management, or relevant papers.

· Designated nurse prescribing qualification (diabetes) or working towards.

· A strong focus on patient assessment.

· Strong leadership skills.

· Adaptability and flexibility.

· Motivation and committed to self-directed learning

· Have excellent interpersonal skills, communication and problem-solving skills in order to relate to and work with staff and patients at all levels

· Be motivated and willing to learn about diabetes disease management in nursing.

· Ability to work within an integrated team

· Maintain safe cultural practice and develop your cultural knowledge
