About the role
Community healthcare provision is key to supporting our health system and community. Southern is going through exciting, innovative changes to provide care to our community. This community clinical coordinator role will support both inpatient and community teams to coordinate efficient, effective and safe services, within available resources.
This role is exciting and varied, it will:
- support teams to get people either home
- to keep people living quality lives at home
- prevent people presenting to hospital, and coordination of interim care into ARC facilities.
You will work alongside:
- community teams
- inpatient teams
- ARC facilities
- primary care agencies
- unit managers
- other clinical coordinators.
This role provides leadership for the coordination of clinical service delivery, clinical leadership and direction to staff across community.
In this role, you will:
- provide clinical leadership
- monitor and mitigate risk
- coordinate ACC contracts
- actively engage in quality initiatives
- manage the day-to-day functioning of the service to ensure delivery of safe patient focused care
- support staff education.
This position does not encompass budget responsibility but has delegation of authority for management of staff within the community team.

We are looking for an experienced healthcare professional who holds a valid annual practicing certificate.
The successful candidate will:
- have strong group facilitation skills and a demonstrated history of a collaborative team approach
- have an excellent knowledge of adult teaching and learning principles
- demonstrate strong clinical decision-making skills
- have excellent communication and engagement skills
- has advanced communication techniques such as conflict resolution, diffusion and mediation skills.
