About the Role
Are you a dynamic leader with vision, drive, and a passion for delivering exceptional patient care? Hauora a Toi Bay of Plenty are seeking an experienced registered health professional, either a Registered Nurse or Allied Health, to lead and implement the implementation of a new service called START (Supported Transfer and Accelerated Rehabilitation Team). START is an early supported discharge model of care in which older people can return to their home following an acute episode or injury to complete their rehabilitation/ enablement at home. The care is provided by trained health care assistants or support workers.
In this senior role, you will lead a team of skilled and dedicated professionals who excel in patient care and provide a mechanism by which older people can return to their homes from hospital faster and stay at home. You will play a pivotal role in fostering collaboration, resilience, and a culture of excellence within the team while ensuring operational safety and evidence-based practices.
The START team will aim to reduce the risk of readmission to hospital through:
- Improving functional resiliency / independence
- Ensuring seamless re-integration back to primary care
- Managing issues related to medication, nutrition
- Managing carer stress
- Identifying deterioration in conditions
- Supervising, managing care packages
A key part of the role will be giving direction and delegation to health care assistants.
