  • Permanent, Part-time 64 hours per fortnight (0.8 FTE)

About the Role

We are seeking a Kaiāwhina to support whaiora across Hoki ki te Kāinga and EngAGE Community Allied Health Service, two community facing services. Hoki ki te Kāinga helps older adults transition from hospital to home, while the EngAGE Service supports people with long-term health conditions and disabilities to remain independent and achieve their wellbeing goals.

 

As Kaiāwhina, you will introduce Māori whaiora and whānau to these services, support referrals, and provide ongoing support for a smooth transition between hospital and community services. Working with Multi-Disciplinary teams, leadership, and Clinical Coordinators, you’ll promote the service and connect whaiora to broader community resources.

 

As a kaiāwhina you will play a pivotal role in supporting our kaimahi and the whaiora and whānau we service.

  • Connecting and providing support to whaiora and their whānau in a culturally responsive and mana-enhancing way
  • Assisting in the delivery of rehab programmes and support services
  • Supporting the development and maintenance of whanaungatanga within the community

 

About the Teams

Both Hoki ki te Kāinga and EngAGE are multidisciplinary teams made up of supportive and skilled health professionals, focused on helping whaiora regain confidence and independence in their own homes.Hoki ki te Kāinga provides essential support for whaiora transitioning home after hospitalization, strengthening whānau involvement in recovery and connecting whaiora with community resources to promote long-term well-being.

 

EngAGE Community Allied Health Service (PT, OT, SW, SLT, Dietetics) work alongside the client and their whānau in collaboration with other healthcare professionals. One part of the EngAGE Service provides a more urgent service for our older people, working in partnership with other departments and service providers to come up with creative solutions for complex situations that can support people to remain living well at home, that helps prevent hospital admissions. The other part of the Service provides routine assessments, treatment and rehab for clients living in our community.

Your experience in the Health and Disability sector, coupled with exceptional interpersonal skills, will help improve health outcomes—especially for our Māori whaiora and their whānau. You’ll be making a real difference in people’s lives.

 

You will have:

  • Strong understanding of tikanga and te reo Māori
  • Experience working with whaiora, whānau, or in a community support role
  • Effective communication skills and a team-oriented mindset
  • Empathetic, reliable, and respectful approach
  • Holds a full NZ driver’s licence
