We have an exciting opportunity for a New Graduate Social Worker to join our friendly team based at Franklin Locality - Pukekohe Hospital. This role will focus on providing Social Work intervention to the inpatient Rehabilitation and Care Ward for Older Adults and supporting the wider Social Work community service, to meet referral demand across all localities
In this position, you will enjoy a diverse range of clinical presentations helping you develop your knowledge and expertise in Social Work Intervention and practise. You will work collaboratively in an interdisciplinary team with a restorative approach to patient and family care. Your skill base will be broad and holistic with problem solving as a key strength. You'll be solving real life challenges on a daily basis, changing for the better the lives of many in our community. Ideally you will be well versed with what the community has to offer our clients. You will have the support of the interdisciplinary team including Advanced Clinician for Social Work and access to ongoing professional development opportunities across the Counties Manukau District. You will have access to regular supervision as per your clinical requirements.

Essential:
• NZ Registered Social Worker with current annual practising certificate
• Passion for working with patients and whanau to improve health outcomes
• Excellent communication skills and ability to work within a team
• IT and Telehealth experience
• Full, clean driver’s license
• Advanced skills and confidence in completing Social Work Psychosocial assessments and intervention.
• Culturally safe practice and commitment to reducing inequity
• Ability to work autonomously
