Health New Zealand | Te Whatu Ora is firmly grounded in the principles of Te Tiriti o Waitangi and is committed to building a health system that serves all New Zealanders.
About the role
The Home Team provides hospital avoidance and supported discharge services, working across a range of settings including the community, Emergency Department, and in-patient areas. This integrated service is designed to respond flexibly to patient and service needs. The team focuses on delivering one car up the driveway, one assessment, and one plan of care.
This is a fantastic opportunity for a registered health professional to join an inter-professional team. In this role, you will support early discharge of suitable patients from Southland Hospital, monitor their recovery, and help ensure they maintain independence at home.
Excitingly, we are also reviewing processes to enable a more responsive community-based service for patients who, while medically stable, require urgent assessment and immediate coordination of services to prevent Emergency Department presentations or hospital admission.
About the Team/Service/Location
Health New Zealand | Te Whatu Ora – Southern has the largest geographical coverage of all the New Zealand regions, providing services to 350,000 people in diverse communities. We take pride in all the services we provide, and our aim is to improve, promote, and protect the health of our region. We believe that our dedicated and passionate staff are a significant driving force behind the achievement of these standards, and we need more like-minded people to help us continue providing for our community.
About you
This role requires someone who is adaptive to change, resourceful, flexible to service needs and future development, and has a genuine passion for working in the community.
You will need the following:
- Comprehensive clinical assessment and problem-solving skills
- The ability to work autonomously and collaboratively within the inter-professional team
- The ability to direct and delegate to rehabilitation assistants
- Excellent communication skills to liaise with health professionals both within inpatient services and in the community
- Recent experience working with older adults in acute, rehabilitation, or community settings
The health professionals in the Home Team currently provide services Monday to Friday, 8.00am - 4.30pm. As part of our commitment to meeting community needs, extended hours and weekend work may be introduced.
You must be a New Zealand-registered health professional with a current APC, have a minimum of two years' post-registration experience in New Zealand, and hold a full, clean New Zealand driver's licence.
To be successful in this role, you will have a strong commitment to ongoing professional development. It is expected that you will either hold or be working towards a postgraduate qualification. Competence in the assessment of complex patient needs is also desired.
