About the Role:
As an HCSS Registered Nurse Assessor, you will use your clinical expertise to
assess the needs of health consumers in community settings and develop tailored
care plans using the InterRAI assessment tool. You - ll collaborate closely with
clients, their families, and the wider healthcare team to ensure the best
possible outcomes for those in your care.
Key Responsibilities:
- Conduct comprehensive clinical assessments using InterRAI to inform care planning.
- Develop individualized care plans and goals for clients receiving home and community-based care.
- Provide one-on-one training and support for healthcare assistants on activities of daily living (ADLs) and home-based care practices.
- Collaborate with the HCSS Associate Lead to support staff development and training initiatives.
- Ensure timely responses to community referrals (within 48 hours where possible).
- Maintain high standards of nursing care, aligning with National and Health NZ policies and the Code of Health & Disability Services Consumer Rights.
- Use a partnership approach to enhance health outcomes for health consumers.
- Participate in quality improvement initiatives and interdisciplinary team discussions.
About the Team/Service/ Location
Our team is made up of clinical and non-clinical staff. We currently have a team of 14 support workers who work directly with clients under the delegation of our nurse assessor. The office team consists of non-clinical Coordinator and clinical Nurse Assessor and Associate Clinical Nurse Manager.

About you
Essential Qualifications & Experience:
- NZ Registered Nurse with a current Annual Practicing Certificate.
- A full, clean NZ driver - s license.
- Strong computer literacy and a willingness to learn new systems.
- Ability to work autonomously while contributing to a collaborative team environment.
- Excellent communication and interpersonal skills - comfortable engaging with people from diverse backgrounds and cultures.
- Cultural sensitivity and understanding of the Principles and Articles of the Treaty of Waitangi.
- High level of self-presentation and professionalism.
- Ability to maintain confidentiality and exercise discretion.
- Capacity to work under pressure and manage competing priorities.
- Commitment to ongoing self-development and professional growth.
- Well-established professional boundaries and accountability within the scope of nursing practice.
Desirable:
- InterRAI competency and knowledge of InterRAI guidelines.
- Previous experience in a community nursing setting.
- Understanding of home and community support systems and service delivery.
